چکیده

INTRODUCTION The rate of fusion surgery of the lumbar spine has remarkably increased over the past 30 years. SOURCES OF DATA We performed a comprehensive search of studies addressing the assessment of sacroiliac joint (SIJ) after lumbar spinal fusion surgery on PubMed, OVID/Medline, Cochrane, CINAHL, Google scholar and Embase. AREAS OF AGREEMENT The fusion of spinal segments leads to degenerative changes in the mobile segments above or below the fused spine, because of the increase of the stresses imposed on the adjacent levels. AREAS OF CONTROVERSY There is no consensus about prevalence and potential risk factors contributing to SIJ degenerative changes in patients undergoing fusion surgery. GROWING POINTS Studies using radiographic evaluation to detect degenerative changes of the SIJ are being performed. AREAS TIMELY FOR DEVELOPING RESEARCH No definitive estimation about the prevalence of degenerative changes of the SIJ after fusion surgery can be provided. Further studies are required to identify the risk factors involved in post-fusion degenerative changes of the SIJ.

Five-Year Clinical and Radiographic Outcomes After Minimally Invasive Sacroiliac Joint Fusion Using Triangular Implants

OBJECT Previous reports of minimally invasive (MIS) sacroiliac (SI) joint fusion for low back, SI joint, and buttock pain secondary to SI joint disorders have shown favorable short- and mid-term outcomes. Herein we present 5-year clinical and radiographic outcomes after MIS SI joint fusion using a series of triangular porous titanium plasma spray (TPS) coated implants. METHODS Consecutive pat...

Chronic sacroiliac joint pain: fusion versus denervation as treatment options

STUDY DESIGN  Systematic reviewObjective: To compare the safety and effectiveness of fusion versus denervation for chronic sacroiliac joint pain after failed conservative management. SUMMARY OF BACKGROUND  METHODS of confirming the sacroiliac joint as a pain source have been extensively studied and reported in the literature. After confirmation of the origin of the pain by positive local anes...

Management of sacroiliac joint disruption and degenerative sacroiliitis with nonoperative care is medical resource-intensive and costly in a United States commercial payer population

INTRODUCTION Low back pain is common and originates in the sacroiliac (SI) joint in 15%-30% of cases. Traditional SI joint disruption/degenerative sacroiliitis treatments include nonoperative care or open SI joint fusion. To evaluate the usefulness of newly developed minimally-invasive technologies, the costs of traditional treatments must be better understood. We assessed the costs of nonopera...

The prevalence of sacroiliac joint degeneration in asymptomatic adults.

BACKGROUND Degenerative changes of the sacroiliac joint have been implicated as a cause of lower back pain in adults. The purpose of this study was to determine the prevalence of sacroiliac joint degeneration in asymptomatic patients. METHODS Five hundred consecutive pelvic computed tomography (CT) scans, made at a tertiary-care medical center, of patients with no history of pain in the lower...

Quality of life in preoperative patients with sacroiliac joint dysfunction is at least as depressed as in other lumbar spinal conditions

BACKGROUND Pain from the sacroiliac joint (SIJ) is an under-recognized cause of low back pain. The degree to which SIJ pain decreases quality of life has not been directly compared to other more familiar conditions of the lumbar spine. METHODS Multivariate regression analysis of individual patient data from two prospective multicenter clinical trials of SIJ fusion and three prospective multic...
